Community Care

St John Ambulance Tasmania Community Care provides quality social support services to Tasmanians who may experience social isolation due to various factors. We aim to enhance the lives of our clients, foster meaningful relationships, and enable them to engage in activities that they find enjoyable and valuable. We assist our clients to maintain their independence, well-being, and social networks and support them to continue to contribute to their community.

Our social support services are delivered by compassionate volunteers, who are supported by a committed team of staff. All our volunteers are carefully selected, police screened, and matched on a one-to-one basis with clients based on skills, experience, and common interests.

Our services

Our Community Care program receives funding from the Commonwealth of Australia represented by Department of Health and Aged Care to deliver aged care activities under the Commonwealth Home Support Program (CHSP) and from the Crown, through Department of Health, to provide Tasmanian HACC Services. The activities we deliver include;

Transport

Our Transport service, helps older Tasmanians maintain their independence by providing safe and reliable transport to essential appointments, social events, and other important activities.

Domestic Assistance – shopping on behalf of client

Our team provides shopping services on behalf of clients who are unable to do so themselves, helping ease the burden of daily tasks for older Tasmanians.

Home maintenance 

We offer basic garden care and maintenance services to ensure our clients' homes are safe and accessible, allowing them to remain in their own homes with confidence.

This service offers one-on-one assistance, including shopping with clients and providing companionship to reduce isolation. Through CHSP funding, we help older Australians stay engaged and supported in their community.

How to access our services

At St John Ambulance, we are proud to offer essential support services for older Australians through two key funding programs: the Home and Community Care (HACC) Program and the Commonwealth Home Support Program (CHSP). These programs help us deliver high-quality care that promotes independence and well-being, allowing people to remain in their own homes for as long as possible. Below, we explain the difference between these programs and how you can access the services.

Commonwealth Home Support Program (CHSP)

The CHSP is a program, funded by the Commonwealth Government, provides a range of basic support services that help people maintain their independence and continue living safely in their own homes. The service is available to people aged 65 years or older, or 50 years or older if you identify as an Aboriginal or Torres Strait Islander person.

How to access CHSP services: 

To access CHSP services, you need to register with My Aged Care, the Australian Government’s portal for aged care services. Following an initial assessment, a referral to CHSP services will be made based on individual needs. For more details, visit the My Aged Care website or call their hotline 1800 200 422.

Home and Community Care (HACC) Program

Funded by the Crown through the Department of Health - Tasmania, the HACC Program is designed to support people who are under 65 (or under 50 for Aboriginal and Torres Strait Islander people) with basic home support services. HACC focuses on providing assistance to younger people with disabilities, their families, and their carers, helping them to live independently in the community.

How to access HACC services: 

To access HACC services, you must obtain a referral from the Tasmanian Community Care Referral Service (TCCRS). To find out if you are eligible to receive HACC services or to create a referral for a service please contact the TCCRS on 1300 769 699.
